This survey describes the use of lay health workers in rural community health centers across the United States. The survey has three purposes, including: to describe how lay health workers are currently being used by rural community health centers; to introduce the lay health worker concept to rural community health center administrators that are unfamiliar with the approach; to provide a preliminary guide to rural community health center administrators who are interested in incorporating lay health workers into their organization. A brief description of the survey application and process is given, as well as background information on lay health workers and a resource section.
